Angelo Catholic School Registration Forms For New Families
Some forms require signtures by parents and/or students. Print them, complete the information, sign, and return to the school, or click here
to request hard copies be mailed. Please specify an address to which the registration forms can be sent.
The Required Registration Forms Checklist New Families (pdf below) will guide you in the items needed for registration. The other forms must be printed, completed and returned to the school along with the registration fee. Your child is not registered, nor his/her place in a class secure until all paperwork is complete and the registration fee paid.
The ACS Website-Marketing Permission Form (pdf below) must be printed, completed, and returned to the school with other registration materials.
In regards to the Diocese of SA Background Check (pdf below), please note, in 2003, the Diocese of San Angelo began requiring all persons, paid or volunteer, who will be in contact with ACS students, to complete a criminal background check and attend an Ethics and Integrity Workshop. It is necessary for your child's safety, as well as the safety of all children, to attend to this matter. ACS needs our parent volunteers. Without you, we will not be able to do as much for your children. Failure to complete the background check and attend one of the workshops will result in NOT being able to volunteer in our programs, including classroom assistance, community service trips, and educational outings.
If you have already had the background check through the Diocese of San Angelo, and you have attended a workshop, please let someone in the office know, so that it can be verified. If not, application forms are available in the office during school hours, or you may print and complete the Diocese of SA Background Check below and return it to the office. Please note: Only the portions marked with an * must be completed by volunteers. The Diocese of San Angelo offers Ethics & Integrity Workshops (please see Diocesan website for specific dates and locations). The Parent-Student Handbook will be available online by August 1. After reading the Handbook, request the form in the office(the form can be emailed), sign, and return no later than Meet Your Teacher.
